On this page, you will discover information about collaborating with families to track child and family progress toward IFSP outcomes by exchanging, synthesizing, and responding to progress information.

What is tracking progress?

Progress information is used to help families and providers make decisions about continuing or adapting the child and family's current outcomes, supports, and strategies.

Part 1: Exchange and Summarize Progress Information

Providers and families collaborate to track progress by exchanging, discussing, and summarizing information about progress toward their individualized family support plan (IFSP) outcomes, progress indicators, and strategy action steps. Progress information is used to help families and providers make decisions about continuing, revising, or identifying new child and family outcomes, supports, or strategy action steps. The 5Q Visual Model, Child Outcome Summary process, and other sources of information can be helpful to use as part of progress tracking discussions and information sharing. 

Part 2: Respond to Progress Information

Responding to progress information requires teams to consider whether to adapt supports or services on the IFSP so the child and family continue to progress toward achieving their desired outcomes. Caregivers and providers also respond to short-term progress information on an ongoing basis using SOOPR caregiver coaching practices and the 5Q framework.
